New Visitors

If this is your first visit to this site, please click the Log In button below and log in with your CAMPUS username and password.

Once you have successfully logged in, you will be presented with a brief form asking for contact information and gender.

Once you have submitted that information, you will be presented with a calendar where you can select the day and time of your appointment. Days that have available appointments will be labled with "Available Appointment". Click on the day that you would prefer, and then click on the appointment that you would prefer. There are two appointments for every 15 minute time block. Simply follow the prompts and you will be registered!

Once registered, you will see the Summary screen. At that point you can change your appointment, cancel your appointment, or print your Consent Form. Please make sure to print that Consent Form and bring it with you to your appointment.

Visually impaired users who use a screen reader may access the "Screen Reader Accessible Form" link on the Calendar page, instead of using the Calendar.

Returning Visitors

Visitors can return to this site at any time to check their appointment, print out their Consent Form, or change or cancel their appointment.
